Welcome to the Medina Valley ISD Health Services Page
Each MVISD campus has a nurse available during school hours. The school nurse is here to support student success by providing health care through assessment, intervention, and follow-up for all children within the school setting.
The school nurse addresses the physical, mental, emotional, and social health needs of students and supports their achievement in the learning process.
If your child has asthma, diabetes, seizures or allergic reactions, the medical forms needed upon enrollment may be found here or obtained from your campus nurse’s office.
If your child will need medication during school hours, a medication form is also available here or with your campus nurse. More information on medication administration procedures can be found in your student handbook.
Healthy kids learn better. The academic success of our students is strongly related to their health. If your child’s health needs change during the school year, please contact your school nurse for any updates to their health plan.

- Tdap (Tetanus, diphtheria toxoids, acellular pertussis)
- MCV4 (Meningococcal)
Whether you are going to your private doctor or the Health Department you should call ahead to ensure that the immunization your child needs is in stock. Please have the doctor or clinic update the shot record and provide it to your school nurse prior to the beginning of the school year.
